The aim of this role is to provide specialist support to improve and maintain the mental health and wellbeing of older adults upon discharge from statutory mental health services.
You will need experience of working with mental health services and supporting vulnerable adults, particularly older people. We are looking for someone who is confident, resilient and compassionate with a strength-based approach.
You must possess a full current driving license with use of a motor vehicle for work purposes.
